The following claims are required:

• Duly filled form for the claim
• Insurance certificate should be the original one along with the policy number
• Copy of the billing lad
• Survey report
• Missing certificate
• Original invoice
• Packing list
• Shipping specification
• Copies of correspondence that has been exchanged
